As a general solution, it's nice to keep the patch, the hook and the peg leg all on the same side for consistency's sake, usually to the leeward. But I know some folks who swear by the alternating arrangement of right/left/right of the various piratical impediments in their standard configuration from top of the figure to bottom to avoid listing, and in that way the rum-swigging hand is free to swig away to squiffiness, leaving the hook hand to be busied with swag, plunder and booty. If you happen to be a lefty, swap your hook to starboard, and simply reverse the corresponding disabilities as needed.
